2010-2011 Open Enrollment Information

IMPORTANT BENEFITS OPEN ENROLLMENT UPDATE

The 2010-2011 Benefit Open Enrollment period has been EXTENDED to noon on Tuesday, September 7, 2010.

Please have all necessary forms returned to the benefits office by this date and time. Employees have three options for submitting all necessary forms:

  • Enrollment forms may be sent via interoffice mail - please allow enough transit time
    OR
  • Drop paperwork off at the District Offices during normal business hours by noon on September 7, 2010 (a benefit associated will review the forms with you)
    Office Hours: 7:45 am – 4:45 pm Mon-Fri
    OR
  • Employees can also attend an Open Enrollment meeting to review and submit paperwork to a benefits associate. Vendors are also available on August 30 & 31 for employee questions.

Open Enrollment Meeting are scheduled as follows:

  • Monday, August 30 - Maroon Rm & Governing Board Rm, 1010 E. 10th Street, 3:00 - 5:30 pm
  • Tuesday, August 3 - Maroon Rm & Governing Board Rm, 1010 E. 10th Street, 7:30 am - Noon
  • Tuesday, September 7 - Governing Board Rm, 1010 E. 10th Street, 8:00am - Noon

Please note: At the MINIMUM, all employees need to complete and submit their enrollment forms and The Centers for Medicare & Medicaid Services Form

Your Individualized Open Enrollment packets and Annual Retirement Benefits Guides will soon be sent to your sites/department!

OPEN ENROLLMENT FROM AUGUST 1 - SEPTEMBER 7 REQUIRED FOR All TUSD BENEFITS:

  • Medical
  • Dental
  • Vision

Due to federal requirements, it is critical that each employee sign the Employee Sign-off Sheet acknowledging that they have received their Open Enrollment packet, which includes the Medicare Part D Notice (found on page 33 in the Benefits Handbook).

The Annual Retirement Benefits Guide is printed by TSA Consulting Group and also delivered to all sites/departments. An employee signature acknowledging receipt of their personalized guide is also a federal requirement.

The Employee Sign-off Sheet must be returned to the TUSD Benefits Office no later than August 31, 2010.

Employees who suffered a reduction in force and have been recalled will receive their Individualized Open Enrollment packet at their site. Your name may not be on the site’s list. Locate your individualized sign-off sheet in your OE packet, sign and date the form and return the completed form to the TUSD Benefits Department.

MEDICAL BENEFITS
TUSD is excited to announce its move to a Medical Benefit self-insurance model with AmeriBen as the Third Party Administrator and Blue Cross/Blue Shield of Arizona as the provider network.

The new available medical plans are:

  • EPO Plan (Exclusive Provider Option) formerly the HMO Plan – this plan represents the base level contribution from TUSD.
  • PPO Plan (Preferred Provider Organization) – same upgrade offered in the past at the employee’s expense unless otherwise provided in the employee agreements
  • POS Plan (Point of Service) – Eliminated – anyone provided this plan at district expense will be offered the PPO plan at district expense
  • HDHP (High Deductible Health Plan)

Although these are big changes for the District, please note that there are NO changes this year to the plan designs of the TUSD Health Insurance options* except that the employee contributions have been increased to reflect the increase in the medical benefit. See your forms for the new rates (effective October 1, 2010). TUSD is fully funding the increases associated with the new EPO plan which replaces the AETNA HMO plan provided in FY 2009/2010.
