    The following paperwork is required for your student to get their iPad. All returning students will only need to fill out the contract for this school year. All new students (the entire 6th grade class and any new 7th or 8th grade students) will need to fill out all of the paperwork below. We are working on making the paperwork submission digital. In the meantime, you're more than welcome to print, sign, scan and submit via email to patricia.vogel@tusd1.org if that would be easier for you.

    Please return soon for this document. Must be filled out bey every student in order to get an iPad. 
  • Parent/Guardian Digital Citizenship Paperwork
    Must be filled out by all new students to Mansfeld (all 6th grade students and any new 7th or 8th grade students)
  • Parent/Guardian End User Participation Agreement
    Must be filled out by all new students to Mansfeld (all 6th grade students and any new 7th or 8th grade students)
  • O365 Account Accessibility Form
    Must be filled out by all new students to Mansfeld (all 6th grade students and any new 7th or 8th grade students)
  • VILs Research Consent Form
    Must be filled out by all new students to Mansfeld (all 6th grade students and any new 7th or 8th grade students)

  • 2020-2021 School Year

    The Verizon Innovative Learning program will continue in the 2020-2021 school year. After filling out the appropriate paperwork, each student will receive an iPad and charger. If your student attended Mansfeld last year and already had an iPad they will need to sign a new contract, but not fill out an entirely new paperwork packet. Check back here and on social media soon for copies of our paperwork requirements for this year. We will be communicating information about the distribution of devices for the upcoming school year shortlyUpdated 7/2/2020
